It is with great compassion and the values of sanguis anima that Dr. Lisa Hutcherson carries out this mission for our residency and fellowship programs as well as teaming with our clinical learning environment sites.<\/p>\n<p>Dr. Hutcherson led an inter-facility team to create and execute the Patient Safety Awareness Room (PSAR) orientation exercise adhering to COVID protocols and little disruption to program specific orientations. This exercise provided an opportunity to access the learners ability to identify patient safety errors in the room and provided live instruction on entering an incident reporting in the reporting systems of the different clinical learning environments. Using a QR code, learners were able to access a pre- and post-survey. 256 learners completed the PSAR room with 196 completing the debrief. Prior to the exercise, 73% were somewhat confident\/not confident in identifying a patient safety issue. After completing the PSAR exercise and debrief, 92% of the learners were confident\/very confident in identifying patient safety issues. 94% agreed\/strongly agreed they knew where and how to report an incident report. 67% of the learners rated the PSAR exercise as very good\/excellent with 27% rating the exercise as good. The learners have been challenged to enter an incident report in a 90 day period. There has been a small increase in reporting early in the timeframe.<\/p>\n<p>Dr. Hutcherson uses other innovative tools such as Mr. Potato Head, building Christmas tree chains, and stacking balloons teaching teamwork and its importance in health systems science. Dr. Hutcherson has mentored a CS&amp;E through a project of improving discharges before 11am in Psychiatry as well as mentoring programs and clinical departments through a root cause analysis bringing about improvements to increase patient safety.<\/p>\n<p>The team that includes Drs. Dyurich, Hutcherson and Prasad from the Office of GME are always a pleasure to work with and and are so helpful.\u00a0 These individuals have worked collectively within the office for GME to provide much needed services for our residents in the realms of mental health and quality and safety efforts.\u00a0 In these efforts, I believe they do their work diligently and quietly and would like to recognize them as these services are really important to the health and wellbeing of our residents and programs. Personally, they have been wonderful collaborators on projects that focus on improving the clinical learning environment.
