{"id":244,"date":"2018-02-06T20:41:52","date_gmt":"2018-02-06T20:41:52","guid":{"rendered":"https:\/\/wp.uthscsa.edu\/ct-surgery\/?page_id=244"},"modified":"2021-11-08T18:28:39","modified_gmt":"2021-11-09T00:28:39","slug":"244-2","status":"publish","type":"page","link":"https:\/\/lsom.uthscsa.edu\/ct-surgery\/patient-care\/congenital-heart\/conditions-we-treat\/244-2\/","title":{"rendered":"Hypoplastic Left Heart Syndrome (HLHS)"},"content":{"rendered":"<div class=\"wpb-content-wrapper\" id=\"wpb-content-root\"><p>[vc_row][vc_column][vc_column_text]HLHS includes a wide spectrum of diseases characterized by a varying degree of underdevelopment of the left-sided heart structures, including the LV, mitral valve, aortic valve, and aorta.\u00a0 The RV thus supports both the pulmonary and systemic circulations.\u00a0 Due to the hypoplastic nature of the aorta, the systemic circulation is dependent upon patency of the PDA (via medical therapy with PGE-1).<\/p>\n<p>Pulmonary venous return mixes in the right atrium via an ASD or anomalous venous connection.\u00a0 Associated non cardiac abnormalities are common (25%), and 5% will have a trisomy syndrome.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-245 alignleft\" src=\"https:\/\/lsom.uthscsa.edu\/ct-surgery\/wp-content\/uploads\/sites\/32\/2018\/02\/families-hlhs.jpg\" alt=\"\" width=\"322\" height=\"257\" srcset=\"https:\/\/lsom.uthscsa.edu\/ct-surgery\/wp-content\/uploads\/sites\/32\/2018\/02\/families-hlhs.jpg 322w, https:\/\/lsom.uthscsa.edu\/ct-surgery\/wp-content\/uploads\/sites\/32\/2018\/02\/families-hlhs-300x239.jpg 300w\" sizes=\"auto, (max-width: 322px) 100vw, 322px\" \/>Neonates may appear normal at birth, but become tachypneic and pale within hours to days, and may progress rapidly to acidosis, cyanosis, and hemodynamic collapse.<\/p>\n<p>Symptoms are dependent upon the size of the inter-atrial communication as this is the only mechanism of getting blood into the systemic circulation (along with a PDA).\u00a0 Patients with severe restriction at the atrial septum will present with respiratory distress, cyanosis, and acidosis immediately after birth (functionally equivalent to obstructed TAPVR) and require immediate balloon atrial septostomy in the cardiac catheterization lab.<\/p>\n<p>CXR shows mild cardiomegaly and increased pulmonary vasculature.\u00a0 Echocardiography is the diagnostic study of choice and provides anatomic details about the aortic valve, aorta, coronary anatomy, atrial septum, and other valvular function.\u00a0 Cardiac catheterization is not routinely performed unless an atrial septostomy is required.<\/p>\n<p>Patients should be treated with PGE1 to maintain ductal patency and supplemental oxygen should be avoided since it causes pulmonary vasodilation and decreases systemic perfusion in the single ventricle child.\u00a0 Surgical treatment is either by staged palliation ending with the Fontan procedure or by transplantation (see above, Tricuspid Atresia and<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-246 alignright\" src=\"https:\/\/lsom.uthscsa.edu\/ct-surgery\/wp-content\/uploads\/sites\/32\/2018\/02\/families-hlhs-2.jpg\" alt=\"\" width=\"292\" height=\"247\" \/><\/p>\n<p>the \u201cSingle Ventricle Pathway\u201d).\u00a0 The first stage in palliation is the Norwood procedure.<\/p>\n<p>The procedure may take place under deep hypothermic arrest.
