Overview:

Any survey of health and illness in the modern world leads to the inescapable conclusion that while disease and death are biological events, their population distribution is strongly shaped by social factors. These conditions can be partially addressed in clinical care through strategic partnerships, but ultimately need policies that increase social equity across all populations. Achieving better outcomes in population health and health care depend on addressing social determinants of health (community level) and social needs (patient level). In this presentation Dr. Robert Ferrer will discuss emerging assessments, interventions, and partnerships at both levels.

Objectives:

After the grand rounds participants will have greater understanding of:
1. Considerations in screening clinical populations for social needs.
2. Emerging evidence from social needs screening programs.
3. Personnel and partnerships to enhance primary care’s linkages to social care.
4. Advocacy and action for community-level change.
