Overview:

Overshot Through the Heart: Calibration and discrimination of the Framingham Risk Score and the Pooled Cohort Equations. We increasingly rely on risk scores such as the Pooled Cohort Equation or the older Framingham Risk Score to make decisions about prescribing statins or aspirin. The Pooled Cohort Equation is recommended by current ACC/AHA guidelines because it is believed to better represent contemporary cardiovascular risk, which has been slowly declining since the Framingham Risk Score was first developed. Both the Framingham Risk Score and the Pooled Cohort Equations significantly overpredicted the five-year risk of a composite cardiovascular event outcome. This is consistent with other research and should give us pause as we use these risk scores to guide therapeutic decision-making. For example, a person with a 10-year risk of 10% probably has a true risk closer to 5%, leading to very different guideline recommendations for a statin.

Objectives:

  • Assess the calibration and discrimination of the Framingham Risk Score and Pooled Cohort Equations in Ontario, Canada
  • Appraise the validity of these risk scores in pre-specified subgroups to gain additional perspectives.
  • Analyze the need for further refinement of cardiovascular disease risk prediction scores.
