Overview:

Transitions of patient care from hospitals, extended care facilities and psychiatric facilities can be very complicated. Ensuring patients receive the medications, equipment, and other support they may need after discharge is imperative for patient safety. In this presentation, Dr. Corona will provide an overview of transitional care management, it’s origin, evolving history and current guidelines to meet CMS standards. Finally, her presentation will discuss how the primary care provider should code visits to capture reimbursement for all the extended services provided.

Objectives:

  1.  Explain the purpose of Transitional Care Management (TCM) and how it originated.
  2.  Identify benefits of structured Transition of Care Management for the adult population.
  3.  Summarize Medicare rules and regulations of Transitional Care Management.
  4.  Identify the contents of Transitional Care Management: (1) Communication, (2) non-face-to-face services, and (3) the face-to-face visit.
