Overview:<\/h3>\n

The Primary Care Behavioral Health (PCBH) model is an expanding approach to the integration of behavioral health services into the primary care setting, and it is associated with improved patient outcomes, lowered costs, and enhanced population health. However, less attention has been paid to the delivery of integrated care within primary care to patients presenting with severe mental illness (SMI), who are more likely to experience challenges accessing adequate primary care services and managing comorbid mental and physical illnesses.<\/p>\n

Objectives:<\/h3>\n

At the end of this presentation, participants will be able to:<\/p>\n

    \n
  1. \u00a0Provide an operational definition of the PCBH model and its key components.<\/li>\n
  2. \u00a0Outline the benefits of integrated care on SMI populations and the primary care teams treating them.<\/li>\n
  3. \u00a0Highlight at least two complex clinical issues that commonly emerge when working with patients with SMI in primary care.<\/li>\n<\/ol>\n
