Community Medicine, in collaboration with <a href=\"https:\/\/www.universityhealthsystem.com\/about-us\" target=\"_blank\" rel=\"noopener\">University Health<\/a>, offers an <a href=\"https:\/\/www.ashp.org\/\" target=\"_blank\" rel=\"noopener\">American Society of Health-System Pharmacists (ASHP)<\/a> accredited <a href=\"https:\/\/www.universityhealthsystem.com\/healthcare-professionals\/residency-programs\/pharmacy-residency\/postgraduate-year-two-ambulatory-care\" target=\"_blank\" rel=\"noopener\">Postgraduate Year 2 Ambulatory Care (PGY2 Amb Care) Pharmacy Residency Program<\/a>.&nbsp; &nbsp;As part of their year-long training, PGY2 Amb Care Pharmacy Residents work alongside Family Medicine residents to provide team-based pharmacotherapy services within the Family Health Center (FHC).&nbsp; Pharmacotherapy services include providing evidence-based patient-centered medication therapy management within the FHC pharmacotherapy clinics, under the supervision of Clinical Ambulatory Care Pharmacy Faculty and the Medical Directors of the respective clinics.<\/p>\n<p>Activities include:<\/p>\n<ul>\n<li>&nbsp;Assessing patient\u2019s medication regimens for indication, effectiveness, safety, and adherence<\/li>\n<li>Identifying, resolving, and preventing medication-related problems<\/li>\n<li>Educating patients and caregivers about medications and chronic illness<\/li>\n<li>Managing chronic illnesses including, but not limited to, diabetes, hypertension, hyperlipidemia, congestive heart failure, CAD, anticoagulation, asthma, COPD, allergic rhinitis, GERD, cirrhosis, hypothyroidism, and osteoporosis through direct patient care under a collaborative practice agreement<\/li>\n<li>Promoting prevention of disease and illness through vaccination, age-appropriate screening, education, and linkage to care<\/li>\n<\/ul>\n<p>PGY2 Amb Care Pharmacy Residents develop skills to serve as a pharmacy subject matter expert and drug information resource for prescribers, clinical staff, and patients.&nbsp; Additionally, PGY2 Amb Care Pharmacy Residents optimize adherence and enhance patient care by creating a&nbsp;comprehensive working knowledge of community resources and&nbsp;pharmacy benefits.&nbsp;&nbsp;PGY2 Amb Care Pharmacy Residents also assist with population health management and quality improvement activities, while educating prescribers and clinical staff on topics of interest that are pertinent to primary care.[\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column width=&#8221;1\/2&#8243;][vc_empty_space]<div class=\"flex-video widescreen\"><iframe src=\"https:\/\/www.youtube.com\/embed\/XqfF3lOPd4A\" title=\"UT Health San Antonio Psychology Internship - Primary Care Track - Program Overview\" allowfullscreen><\/iframe><\/div>[\/vc_column][vc_column width=&#8221;1\/2&#8243; In addition to functioning as a Behavioral Health Consultant, duties also include assisting in the supervision of family medicine residents, medical students, and psychology residents\/externs. Candidates will also have the opportunity to engage in a quality improvement\/research project during their training year. Candidates with interest and\/or experience in consultation-liaison services within an inpatient medical setting are also encouraged to apply as the position can include time working with patients and trainees on the inpatient Family Medicine service. A minor rotation (up to 20% FTE) with the Pediatric Hematology\/Oncology outpatient clinic will be incorporated into the fellow\u2019s learning plan to support the professional development towards a well-rounded health service psychologist in primary care.
