- Pre-paid specimen containers are available through the Oral Pathology division office, (210) 567-4073 or (210) 567-1406. These mailing tubes have a bar-coded return address label and contain a laboratory requisition, specimen bottle, and specimen bottle label. All specimen bottles have appropriate warning labels concerning formaldehyde.
- Label all specimen fixative vials with patients name, date of birth, and doctors name.
- Fill in all information on the requisition form including the billing/insurance information, doctor name and address and place the requisition form in the mailing tube with the specimen.
- Additional containers are mailed to dental offices when specimens are received. One new container sent for each specimen received in the laboratory.
