Please include, along with the specimen, a completed Flow <\/span>Requisition<\/span><\/a> Form containing:<\/span><\/p>\n

    \n
  1. Physician’s name, phone number and preferred mailing address.<\/span><\/li>\n
  2. Hospital information (i.e. patient’s ID number, sex, date of birth etc.)<\/span><\/li>\n
  3. Provisional diagnosis<\/span><\/li>\n
  4. Any pertinent patient history.<\/span><\/li>\n
  5. Billing information<\/span><\/li>\n<\/ol>\n

    Please call the lab weekdays 8:00 a.m. to 5:00 p.m. at 210-450-8345<\/strong><\/a><\/p>\n

    Or\u00a0call Client Services after hours or on weekends<\/strong> at 210-567-6599 <\/strong><\/a>with any questions.<\/p>\n
