The below are some examples of records to be sent prior to the evaluation (if available):<\/p>\n<ul>\n<li>Signed court order for the CME (for assigned court cases)<\/li>\n<li>Application for guardianship with any information about the underlying concerns<\/li>\n<li>Medical Records from current physicians.
