Instructions for Bequeathing Body

Please print the Body Bequeathal Agreement. Fill it out, sign it having your signature witnessed by two people over 18 years of age. Then send form to the Body Donation Program at the address on letterhead of the form. Upon receipt of the form by the Body Donation Program, it will be signed by the coordinator and a copy of the form along with an identification card will be sent to you.
