Services Provided

The Transitional Care Clinic (TCC) is dedicated to improving the quality of peoples’ lives by engaging them in mental health services, promoting growth toward personal goals, decreasing the need for psychiatric hospitalization, and linking them to long-term services.

The TCC is a short-term clinic that helps people transition from psychiatric care at a local hospital to long-term mental health care in the community. The TCC provides medication management, individual and group counseling (or psychotherapy), and care coordination (social work) services. Appointments are required, and services are by referral only. Referrals are accepted from designated staff at San Antonio hospitals via the Monarch online referral system.

The TCC typically works with people for around three months while helping them connect to long-term psychiatric care and counseling in the community. People are discharged from the clinic when they begin psychiatric treatment in a different clinic.

If a TCC patient is hospitalized due to psychiatry symptoms, they are automatically discharged from the clinic and should return only if referred again by the hospital.
