Overview:

In an effort to honor the needs of our geriatric patients, Dr. Neela Patel- Chief of Geriatrics & Supportive Care for UT Health San Antonio will speak about what it means to be an Age-Friendly Health System and how Family Medicine clinicians can address what matters. Becoming an Age-Friendly Health System requires that clinicians provide a set of four evidence-based elements of high-quality care, known as the “4Ms,” to all older adults in our system: What Matters, Medication, Mentation, and Mobility. In this presentation Dr. Patel will discuss the “4Ms,” define Advance Care Planning and Advance Directives. Insight will be provided directly from Dr. Patel’s practice on when, where and how to start meaningful discussions with your patients, and identify how to make shared decisions. The CARIÑOS approach will elucidate how shared decisions can be successfully fused in daily clinical practice. Participants will understand the impact advance care planning has on practice and the lives of patients.
