Grand Rounds – AAFP POEMs: SGLT-2 Inhibitors and GLP-1 Receptor Agonists Reduce All-Cause Mortality Amongst Other Patient Benefits

Overview:
Brief review of pharmacotherapy of GLP-1 RA and SGLT2-I reminds providers that GLP-1 RA work by activating insulin synthesis and release, delaying gastric emptying, and increasing satiety. SGLT-2 inhibitors work by reducing renal tubular glucose reabsorption, increasing urinary glucose excretion, ultimately reducing serum glucose with additional weight loss and antihypertensive benefits independent of beta cell function. Review of two separate meta-analyses conclude that SGLT2-I therapy and GLP-1 RA therapy result in reduction in all-cause mortality, CV mortality, myocardial infarction, and adverse renal outcomes. Call to action for physicians and providers to routinely evaluate need for use of SGLT2-I in every patient with cardiometabolic or renal disease, not just diabetic patients, and to work closely with pharmacy teams to assist patients with medication cost barriers.
Objectives:
- Summarize the pharmacology of available GLP-1 receptor agonists and SGLT-2 inhibitors.
- Analyze two meta-analyses evaluating the effect of SGLT-2 inhibitors and GLP-1 receptor agonists on patient outcomes and patient mortality.
- Promote increased use of SGLT-2 inhibitors and GLP-1 receptor agonists in the primary care setting.
