Overview:

Chronic Obstructive Pulmonary Disease (COPD) is the fourth leading cause of death in the United States and Primary Care providers play a key role in the diagnosis and treatment of this disease.  In this presentation, Dr. Hamilton-Bevil will discuss the systematic review with meta-analysis of pharmacologic therapies used for 10,758 patients from 68 randomized controlled trials with COPD exacerbation. There is strong evidence for use of antibiotics and systemic corticosteroids in improving dyspnea and reducing treatment failure for adults with COPD exacerbations, ranging from mild to severe. Other treatments that are part of our usual care, including use of short-acting bronchodilators, did not have clear evidence for resolving exacerbations nor lowering rates of treatment failure.

Objectives:

  1.  Summarize  various pharmacologic therapies used in treating COPD exacerbations and explain their comparative effectiveness.
  2.   Identify evidence that supports use of antibiotics and systemic corticosteroids for COPD exacerbations, independent of setting or severity.
  3.  Discriminate among therapies have insufficient evidence for successful treatment as well as adverse effects.
