Sending the Sample
Specimens (See Specimen Requirements) need to be sent protected from heat to:

Please include, along with the specimen, a completed Flow Requisition Form containing:
- Physician’s name, phone number and preferred mailing address.
- Hospital information (i.e. patient’s ID number, sex, date of birth etc.)
- Provisional diagnosis
- Any pertinent patient history.
- Billing information

NOTE: For prompt processing, the specimen should be received by 4:00 p.m. on weekdays.
