All training verifications requests must be submitted in writing indicating the resident’s or fellow’s full name, dates of training, specific information being requested, and a signed authorization of release form from the resident or fellow.

State Licensing Boards:
- Verifications received directly from state licensing boards are exempt from the administrative service charge
- Verifications received directly from credentialing agencies are subject to the $50.00 administrative fee

*Fees will be waived for two years after graduation from their residency/fellowship program
