UT Primary Care Center (PCC)
The UT Health Physicians Primary Care Center (PCC) has multiple locations in and around San Antonio, TX. Faculty work in multi-disciplinary teams to deliver high-quality patient-centered primary care services to a wide range of patients. Faculty in the PCC have opportunities to learn quality improvement tools, conduct multi-disciplinary quality improvement projects, teach medical students, and mentor residents.
Robert B. Green General Medicine Clinic (GMC)
The General Medicine Clinic (GMC) is a collaboration of providers from UT Health San Antonio and staff from University Health System. At the GMC, faculty see patients in their own faculty clinics and as preceptors of internal medicine residents. We offer consultation services in geriatrics, joint injections for knee and shoulder pathologies, and management of resistant hypertension. We emphasize preventive services such as cancer screenings and management of chronic medical conditions such as diabetes and heart failure. Our team includes a nurse practitioner for co-management with our physicians and a nurse case manager for our complex patients.
Audie Murphy VA Internal Medicine Clinic (IMC)
The VA Internal Medicine Clinic (IMC) is an interdisciplinary clinic grounded in the principles of the patient-centered medical home. There are resident Patient-Aligned Care Teams (PACT) and mini-teams. Each PACT is led by a faculty member who has an appointment in the Division of General Medicine. The interdisciplinary teams are staffed with an RN, LVN, and medical administration support and share a social worker, pharmacist, nutritionist, diabetes mellitus nurse educator, and psychologists. At the IMC, faculty precept residents and see patients in their own longitudinal primary care clinics. IMC offers consultation services and post-graduate medical education opportunities in its Preoperative Clinic and in musculoskeletal medicine through the Sports Medicine Clinic. IMC offers same day access to primary care and behavioral health. Bimonthly meetings are held with other VA primary care clinics in the San Antonio area to continuously improve processes and outcomes.
