NEISD UT Health School Virtual Care Registration

A call from your child’s school nurse no longer means you must leave work immediately and try to squeeze your child into a last-minute doctor’s appointment or urgent care clinic. The UT Health School Virtual Care program allows your child to see one of our board-certified pediatricians or a certified nurse practitioner during school hours while in the school nurse’s office. Preregistration is required, so register today!

To complete registration for the UT Health School Virtual Care program. Please complete each section. All three forms must be completed to be completely registered for the program.

• Registration
  • Guardians and Parents must submit student information, guardian information, and other essential details to initiate student registration.
• Telehealth Consent
  • The Telehealth Consent gives the student permission to participate in the program.
• My Chart Access
  • This form creates a Patient Account on MyChart, the UT Health Patient Portal. Please have your ID and other guardian information available to complete this form. Guardians can pay co-pays and find doctor notes, and all medical information can be found through the patient portal.
